Some young people don’t fit neatly into existing services. Neither should the solution.
Nurture’s Intensive Community Support Services bring together specialist expertise, clinical insight and responsive community provision around the individual, creating the conditions for stability where traditional pathways have struggled. We work beyond organisational boundaries to turn fragmented care into one coherent direction, helping reduce crisis, strengthen the people around the young person and build a more sustainable future.
Our Nurture Community Support Services are designed for young people and young adults whose needs may span health, social care, education, neurodevelopment, safeguarding, behaviour, family relationships and accommodation.
We support Local Authorities, NHS commissioners and partner organisations where needs have become too complex for conventional models, placements are at risk of breaking down, or a young person needs something more individually designed.
Our multidisciplinary expertise can bring together clinical leadership, psychology, mental health, occupational therapy, communication and neurodiversity expertise, education, therapeutic input, workforce development and specialist support. Where accommodation is part of the challenge, we can work with commissioners to shape a solution around housing as well as care and support, creating an environment capable of supporting the wider plan rather than expecting the individual to fit an unsuitable setting.
The ambition is bigger than maintaining a placement. It is to create stability, belonging, meaningful daily life and a credible pathway into adulthood.
Our offering:
There is no off-the-shelf version of Intensive Community Support.
We start with the outcome that needs to be achieved and build the right level of support around it, from strengthening an existing provider or placement, through to creating a dedicated specialist workforce or a more comprehensive community-based solution.
